Instructions

Welcome to Phase 3 of this indexing project.

After you have initially registered and gone through the login page, you will have the option to begin indexing. But, before you register, let's go over the instructions.

Your task will be to assign the following information to every image:

  • Last name
  • First name
  • Middle name (or initial)
  • Date of death
The images are normally one individual per record. Though, if there were receipts or other loose papers involved with the record, they were photographed separately on top of the original record. You will also index these.

DO NOT enter information that you cannot find on these images (for example, you might be tempted to check the death index to find a missing date). Also, please DO NOT guess. Enter only what you find on these images.

    NOTE: Enter ALL first, middle and last names EXACTLY as they are shown on the record. 
Guidelines:
  • Some of the individuals were in the military, were doctors, etc. and/or have titles in their names. For example, Dr., Maj., Gen. Lt., etc. These titles are not to be included. Also, if they were John Doe II or III, or Jr. or Sr., those suffixes (II, Jr., etc.) are also not to be included. 
  • Some of the records are for children and may be indicated as "child of", "daughter of", "son of", "stillborn of", "unnamed", etc. Use the firstname/middlename if they are given, otherwise use ONLY "child" as the first name.
  • Do not enter periods (".") or commas (","). 
  • Your goal is to determine only ONE first name and ONE last name for each individual (unless they are clearly hyphenated). Two or more names may be entered in the middle name field. 
  • If there is NO first name or NO last name, flag it as "unknown." It is okay if there isn't a middle name or initial.
  • Occasionally, there will be an alias, maiden name, nickname, or what you may think is the correct name listed for an individual. You will have an option to indicate an “alias” exists for that file. 
  • If the date-of-death is not exactly indicated, enter the earliest date on the record. Otherwise, flag it as "unknown."
  • If for some reason there is no image, the image is completely blank, or it obviously isn't a record you will have the option to flag it as “unknown.”
Finally, please note that we will make two passes through the current set. This allows us to check for accuracy.
Once again, if you just want to do a search, please go to San Francisco Mortuary Records. You cannot search within this project.
